A report into the fatal shooting of Halyna Hutchins by Alec Baldwin on the set of Rust has discovered that the management of the film "knew that firearm safety procedures were not being followed."

New Mexico workplace safety regulators have issued the maximum fine against Rust's film production company. The penalty imposed is $139,793 (£107,019).Alec Baldwin was pointing a gun at Hutchins prior to the filming of a scene for the western in New Mexico on the 21st October when it went off, killed Hutchins and wounded the films director, Joel Souza.Environment Cabinet Secretary James Kenney commented that:

"Our investigation found that this tragic incident never would have happened if Rust Movie Productions, LLC had followed national film industry standards for firearm safety."this is a complete failure of the employer to follow recognised national protocols that keep employees safe."Bob Genoway, bureau chief for occupational safety, told The Associated Press that: "What we had, based on our investigators' findings, was a set of obvious hazards to employees regarding the use of firearms and management's failure to action upon those obvious hazards."The report would detail how the company "knew that firearm safety procedures were not being followed on set and demonstrated plain indifference to employee safety."

Alec Baldwin is currently facing several lawsuits, including those brought by Mamie Mitchell (script supervisor), Serge Svetnoy (head of lighting) and Ms Hutchins' family.The new report confirms that a large-caliber revolver was handed to Baldwin by David Halls, an assistant director, without talking with on-set weapons specialists either during or after the gun was loaded.The report noted that: "The Safety Coordinator was present on set and took no direct action to address safety concerns."Hall, who served as the safety coordinator, witness two accidental discharges from rifles on set, however, neither he nor the other managers who were aware of these incidents took any investigative, corrective or disciplinary action.
